An intercollaborative initiative by the Centre for Studies in Aging & Health [CSAH] in Kingston, Ontario, the Geriatric eToolkit is an interprofessional resource developed to help direct care providers deliver comprehensive and expedited evidence-based geriatric care.
The tools are organized into four categories:
• Screening tools
Simple screens that flag the need for further assessment
• Assesment tools
More detailed assessment of single issues
• Comprehensive Assessment tools
Thorough or holistic assessment of one or more issues
• Decision Support tools
