The Yukon Home Care Program provides health services to individuals who cannot easily access health care in the community, or who need assistance in performing daily activities. The program aims to improve people’s quality of life and supports them to live independently in their own homes. Our Home Care program and Palliative Care team are involved with all Yukoners requiring palliative care.
